1. Performs and records history and physical examinations and records data from clinic visits.
2. Evaluates, consults, develops diagnosis, and manages the primary care panel of patients assigned.
3. Enters progress notes in computerized medical record and completes workload documentation in a timely manner.
4. Provides medical care encompassing acute and chronic medical conditions and all appropriate preventive care for both male and female veterans.
5. Orders diagnostic tests, medications, laboratory tests as medically relevant for each patient.
6. Provides timely follow-up on results and patient inquiries, including and not limited to completion of view alerts, secure messages and other patient communications.
7. Determines medical assessment and develops treatment plans according to accepted protocols.
8. Prescribes medications according to approved formulary, and in accordance with Federal and State Law.
9. Educates patients concerning medical conditions and recommended treatments. (Specific topics include but are not limited to nutritional counseling, need for smoking cessation, need for periodic immunizations and health screening tests, acute and chronic disease management and other preventive health measures).
10. Consults other specialties as appropriate, according to service agreements.
11. Care of female patients may be required as part of ongoing duties both on an ad hoc basis and possibly as a Women's Health Designated Provider.

This position utilizes knowledge of policies and procedures for delivery orders and small purchases and knowledge of commercial supply sources and common business practices related to sales, prices, discounts, units of measurement, deliveries, stocks, and shipments. The Ralph H. Johnson VA Medical Center (RHJVAMC) is recruiting for one PACT Clinical Pharmacy Practitioner (CPP) at the Beaufort, SC Community Based Outpatient Clinic. The PACT CPP is responsible for planning, implementing and operating the clinical pharmacy programs and is assigned to cover in the primary care or any of the specialized areas of pharmacy. The CPP plays a defined role in budgetary execution and serves as a mid-level provider to initiate, modify, or discontinue medication therapy, except for controlled substances, and as a consultant for medication services by the providers of the medical center. The CPP is primarily responsible for providing direct patient care to scheduled and unscheduled patients through referral, warm handoff and population management. Duties include but not limited to:

• Accepts a shared responsibility in the development and ongoing review of all policies and operating procedures specific to clinical practice area.
• Responsible for the appropriate, effective and safe use of pharmaceuticals within practice area. Initiates quality assurance and improvement activities, including but not limited to medication use evaluations, medication management, monitoring and safety, criteria development, and policy creation and/or revision, that demonstrate improved patient care outcomes.
• Contributes to budgetary stewardship by incorporating cost savings initiatives into clinical practice and identifying potential cost reduction opportunities to improve overall healthcare services.
• Independently resolves provider and patient care issues, concerns, and questions as related to clinical practice.
• Provides clinical coverage in the pharmacist-run clinics in Primary Care and other clinics as assigned.
• Offers expert knowledge and advance skills in pharmacology, therapeutics, drug-drug interactions, and medication safety.
• Reviews clinic patient profiles for appropriate drug therapy, adherence issues, drug interactions, adverse drug reactions, medication safety, unnecessary polypharmacy, and dosage adjustments for special considerations such as renal/hepatic insufficiency.
• Obtains patient medication histories and counsels patients and/or relatives on medications as requested by providers or for patients enrolled in pharmacist-run clinics.
• Evaluates and processes non-formulary and restricted medication requests from assigned providers in a timely manner, and adheres to policies and procedures related to the processing of these requests.
• Identifies, reports, and appropriately manages medication-related complications, adverse drug reactions or events, and medication errors.
• Provides expert, clinically appropriate drug information to patients, caregivers & medical center staff as needed.
• Orders laboratory tests as necessary to evaluate and monitor medication therapy.
• Demonstrate the knowledge and skills necessary to provide care appropriate to adult and geriatric patients in his or her assigned unit, area or department.
• Maintains clinical experience, expertise, and scope of practice in areas consistent with clinical background.
• Effectively communicates with providers to establish positive interprofessional relationships, influence clinical care decisions, and solve technical issues related to clinical practice independently as needed.
• Orders supplies to support medication compliance and accurate medication administration (e.g., syringes, needles, tablet splitters), in accordance with VA medical facility policy.
• Manages the clinic automated dispensing cabinet (Pyxis) inventory, dispenses urgently needed medications in clinic as needed in collaboration with PACT teams and assist with contract pharmacy prescriptions if needed.
• Ensures proper storage and monitoring of refrigerated medications in clinic, including management of any temperature excursions.
• Successfully functions as a member of the pharmacy team, which may involve interacting with and/or training clinical pharmacy technicians, clinical pharmacists and other clinical pharmacy practitioners in specialty and primary care sections.
• Maintains adequate qualifications to precept APPE pharmacy school students and/or pharmacy residents (PGY1 and PGY2) successfully through providing constructive feedback regarding quality of educational experiences.

• Provides professional fitness training to authorized patrons to include: Active Duty, Retirees, Family Members and Department of Defense (DoD) Civilian employees at a large command.
• Organizes and conducts the full range of physical fitness assessments to include but not limited to: screening and risk stratification, measurement and evaluation of an individual’s aerobic capacity, body fat percentage, muscular strength, muscular endurance, flexibility, blood pressure, functional movement screens, and interpretation of assessment results.
• Provides individualized and group exercise plans.
• Analyzes physical fitness testing results and data as compared to norms.
• Provides instruction and consultation on proper exercise techniques to individuals and groups, and implementation and maintenance of a comprehensive fitness program to improve aerobic fitness, muscular fitness, body composition, flexibility, and functional movement.
• Instructs patrons in proper use of a wide range of exercise equipment while training, and how to self-monitor progress either verbally or by composing written instructions for use of equipment, safety and well-being.
• Coordinates with and maintains schedules for command representatives to develop and implement physical fitness programming for Active Duty military personnel.
• Level 2 Specialist with the (TSAC) Tactical Strength and Conditioning and (USAW) United States Weight Lifting certification. Will be tasked with leading and assisting in Unit PT sessions, teaching group exercise classes, instructing HITT Courses, Employee Wellness Program, Semper Fit briefs, seasonal fitness programing and any other related programs that are conducted MCCS.
• Provides personal training services to varied population regarding age, medical status, and physical capabilities.
• Incumbent will work with populations deemed to be healthy and special populations consisting of; clients currently under a physician’s care or those who have a medical referral.
• Under the guidance of a supervisor, be able to adapt physical fitness assessments and exercise programs based on these variables.
• Researching training programs that reflect industry best practices and compliance with Marine Corps policies and regulations, to include conferences and workshops on changing technology.
• Researches, and benchmarks emerging trends in the physical fitness industry and observes and reports on potential impacts on the Marine Corps. Reports on performance of programs to higher authority.
• Effectively markets services, programs and events using all available resources.
• May register patrons, collect exercise fees, and maintain attendance and appropriate accounting records.
• Assists in cleaning and maintaining exercise equipment in proper condition. Assist in all fitness center operations.
• Provides World Class Customer Service with an emphasis on courtesy. Assists customers and communicates positively in a friendly manner. Takes action to solve problems quickly. Alerts the higher-level supervisor or proper point of contact for help when problems arise. Adheres to safety regulations and standards.
